The Compassionate Communities programme, commissioned by Aneurin Bevan Heath Board, runs alongside the Integrated Wellbeing Networks programme, complementing each other as part of the CWTSH network.
Compassionate Communities is based on elements of the Health Connections Mendip programme. Working with Caerphilly CBC Social Services - and also Blaenau Gwent CBC in its local authority area - a major element of the programme is connecting and supporting GP surgeries and primary care with the wider community activities.
Along with supporting enhanced connectivity within the surgeries themselves, a key feature is the additional support through Community Connectors (click to find out more).
Community Connectors have been working successfully out of the Information, Advice and Assistance (IAA) service within Caerphilly CBC Social Services. IAA is the "front door" to accessing services from the authority's adult service and children's service divisions.
Through the Compassionate Communities funding, additional Community Connectors are now working in the upper Rhymney Valley with at least one allocated to each of the surgeries (with the exception of Gelligaer Surgery at this time).
